For Referring Physicians

Dr. Lakhiani welcomes referrals from physicians across the country for patients with complex nerve, reconstructive, and microsurgical needs. No formal paperwork required — a phone call or patient request is all it takes.

Thank you for your trust in referring patients to our practice. Dr. Lakhiani and his team are committed to providing thorough, expert evaluation and communicating findings back to referring physicians in a timely manner. We take great care to serve as true partners in the management of your most complex cases.

How to Refer a Patient

The referral process is simple and flexible. There is no formal referral paperwork required. Patients may be referred in one of two ways:

  • Call our office directly at (732) 200-2531 to discuss the case or schedule on behalf of your patient.
  • Direct the patient to request an appointment online at advancedreconstruction.com. The patient can note your name as the referring physician at time of request.

While no paperwork is required to initiate the referral, having prior records and imaging available before the first visit allows Dr. Lakhiani to provide the most thorough evaluation possible.

What to Send

The following records are most helpful, though the absence of any item should not delay referral:

  • Prior operative and procedure reports
  • Imaging studies — MRI, CT scan (with or without contrast, as applicable)
  • Electrodiagnostic studies — EMG and nerve conduction studies (NCS)
  • Relevant office notes and consultation reports
  • Current medication list

Records may be faxed to our office or uploaded through the patient portal. Our staff can assist referring offices with secure transmission.

Conditions We Accept Referrals For

Dr. Lakhiani accepts referrals across a broad range of nerve, reconstructive, and microsurgical conditions, including complex and revision cases:

  • Pelvic pain / pudendal neuralgia / piriformis syndrome
  • Facial paralysis requiring reanimation
  • Peripheral nerve injuries and compressions (upper and lower extremity)
  • Lower extremity nerve compressions (common peroneal, tarsal tunnel, and related syndromes)
  • Complex wounds requiring free flap reconstruction
  • Failed prior nerve surgery and revision cases
  • Limb salvage and limb-threatening wounds
  • Sternal wound infections after cardiac surgery
  • Head & neck cancer reconstruction
  • Amputation care — targeted muscle reinnervation (TMR), regenerative peripheral nerve interface (RPNI), and stump revision

If you are uncertain whether a patient's condition falls within our scope, please call the office. We are happy to discuss cases informally before a formal referral is made.

What Patients Can Expect

At the first visit, Dr. Lakhiani will conduct a thorough history and physical examination, review all available records and imaging, and engage in a detailed conversation with the patient about their diagnosis, treatment options, expected outcomes, and risks. Patients leave the consultation with a clear understanding of their condition and a defined plan of care.

Following the visit, Dr. Lakhiani's office will send a summary of findings and the proposed treatment plan to the referring physician. We are available by phone for any follow-up questions.

Multidisciplinary Clinics

For certain complex conditions, Dr. Lakhiani participates in dedicated multidisciplinary clinics that bring together specialists for a coordinated evaluation:

  • Pelvic Pain Clinic — Brings together Dr. Lakhiani with physiatrists (PM&R physicians), pelvic floor physical therapists, and urologists for a comprehensive evaluation of chronic pelvic pain, pudendal neuralgia, and related conditions.
  • Facial Nerve Clinic — A collaborative clinic with Dr. Dayan for patients with facial paralysis requiring surgical reanimation.
  • Lower Extremity Salvage Clinic — Coordinated care for patients with limb-threatening wounds, complex lower extremity injuries, and amputation-related needs.

Patients referred to these clinics benefit from a single coordinated visit rather than multiple separate consultations.
